1. Please use the exact name as shown on the birth certificate.
2. When choosing a program, click "Travel".
3. Player been rostered to a team in USYS in travel for 2010-2011? answer "No".
4. Number of years played, use an approximate amount.
5. Be sure to fill out all information and add your current email address.
6. Be sure to check your information before you click "yes" to validate.
